Dr. Abraham Jacob, MD, FACC is a board-certified cardiologist with more than 20 years of experience caring for patients with cardiovascular disease. In many cases, a referral is not required. However, patients with HMO insurance plans typically do need a referral from their primary care provider (PCP). Because insurance requirements can vary, we recommend contacting your insurance provider or our office for assistance before scheduling your appointment.
When should I see a cardiologist?
You should consider seeing a cardiologist if you are experiencing symptoms such as chest pain, shortness of breath, dizziness, palpitations, or unexplained fatigue. A cardiology evaluation is also recommended if you have a history of heart disease, high blood pressure, high cholesterol, diabetes, or a family history of heart conditions. Even without symptoms, preventive heart screenings can help identify potential concerns early—before they become serious.

What should I bring to my appointment?
Please bring a valid photo ID, your insurance card, and a list of current medications, including dosages. If you have recent test results, imaging, or referral paperwork, bringing those documents can be helpful. Arriving a few minutes early allows time to complete any necessary paperwork and helps ensure a smooth visit.
